Our Required Booking Forms

Travel Agreement / Travel Information Forms:  To confirm your travel reservations with Eyes on Africa, please complete the Travel Information Form and sign the Travel Agreement and return them to us with your deposit. Both forms must be completed.

A third form, the Travel Insurance Form, must be completed, signed and returned to us at or before the time of your final payment.

Our forms are available in PDF format may be filled in by typing your information in directly via your computer.  Alternatively, you may print the forms and complete them by hand. If you need help, go to our Travel Forms Instructions.

The Travel Agreement and Travel Information Form must both be completed to secure your travel reservations with Eyes on Africa. If unsure about how to complete the forms and agreement, please read below for detailed instructions.

Forms of Payment: Payments may be made by personal, business or certified check, bank transfer (ask us for our bank details) or by credit card.

Eyes on Africa uses PayPal to process credit card payments.

PayPal accepts American Visa, MasterCard, American Express, and in some cases, other cards as well.

To charge any portion of your trip fare to a credit card, please complete the Credit Card Charge Authorization Form and return it to us via fax, mail, or scan/email.
